Hancock Health is making it easier to access your patient portal with our new free app for Apple and Android devices! To get this time-saving app, visit the App Store on your iOS device or the Google Play store on your Android OS device and type “Meditech MHealth” in the search bar. After downloading and installing Meditech MHealth, simply launch the app and click on Hancock Health.
If you are not already enrolled in HealthConnect, what are you waiting for? HealthConnect allows you to view your medical records, visit summaries, lab results, and test results. It also gives you the capability to request prescription refills and send messages to your Hancock Physician Network care providers. To get started, call the HealthConnect Help Line at (317) 468-6060 or send an e-mail to email@example.com.
You can also have family members’ patient portals connected to your portal! This allows you to view medical records for your children, significant others, parents, etc., without having to create separate log-on information for each member. A proxy form must be completed and signed by the portal user and the family member (if they are 12 years old or older).
Proxy forms are available by calling (317) 468-6060, e-mailing firstname.lastname@example.org, or from Health Information Services (HIS) located at the hospital. Once a proxy form is completed for each family member, the completed form may be submitted by scanning and e-mailing to email@example.com or faxing to (317) 468-6267. The form can also be dropped off at any Hancock Physician Network office and HIS, or mailed to:
