Referral and Admission Process

For admission, patients must have clearly documented medical conditions that require daily monitoring and/or intervention by a team of healthcare professionals. When you are referred to us:

  • An admission coordinator schedules an onsite preadmission assessment within 24 hours of referral.
  • A clinical liaison conducts an assessment and confers with the patient, the patient’s healthcare provider, referring physician, and family to determine the patient’s functional ability and desired goals.
  • The referral source is notified of the patient’s acceptance or denial for admission within 24 hours.
  • The admission coordinator schedules the admission with the referral source.
