You have the right to request your medical records from Hancock Health. As of April 14, 2003, all healthcare providers are required by the Health Insurance Portability and Accountability Act (HIPAA) to allow you access to your medical records. Under this law, you also have the right to see, get a copy of, and make a request for a correction to your medical record. Healthcare providers are also allowed to charge a reasonable fee for copy record.
You can mail or fax us your completed authorization for release of medical records. We can mail back your requested records, but can fax them only to other healthcare facilities. If you have further questions, please don’t hesitate to contact us.
